The lungs are hyperinflated with relatively flat diaphragms - a sign of pulmonary emphysema. there is a dense triangular opacity overlying the cardiac shadow with increased lucency of the left upper zone relative to the right upper zone. this is the "sail sign" of left lower lobe collapse with subsequent left upper lobe hyper-expansion.. There are numerous sails signs in radiology, where a normal structure is displaced or a pathology creates the appearance of a sail:. elbow sail sign: the raised anterior fat pad on an elbow radiograph; thymic sail sign: normal thymus on a pediatric chest radiograph; spinnaker-sail sign (angel wing sign): thymus outlined in pneumomediastinum; retrocardiac sail sign: left lower lobe collapse on. The spinnaker sign (also known as the angel wing sign) is a sign of pneumomediastinum seen on neonatal chest radiographs. it refers to the thymus being outlined by air with each lobe displaced laterally and appearing like spinnaker sails. this is distinct from the sail sign appearance of the normal thymus.. see also. thymic notch sign; thymic sail sign; thymic wave sign.
